DOT Physical Pricing

A DOT physical is $89 at our current promo rate. That covers the full FMCSA exam and your medical examiner's certificate. The other three services below are only added if your exam or your employer calls for them, and you will know the price before anything is done.

How It Works

Call to Schedule an Appointment

Call (909) 727-3911 to set up an appointment at the location closest to your route. Appointments are required — walk-ins are not accepted. Bring your license, your glasses or hearing aids, and your medication list.

Complete Your Exam

A certified medical examiner reviews your history, checks your vision, hearing, blood pressure, and urine sample, and completes the physical. Plan on about 30 to 45 minutes.

Leave Certified

When you pass, you walk out with your medical examiner's certificate. We file your paperwork the same day as your exam, so there is nothing left for you to chase down.

If Your Exam Finds a Problem, We Treat It Here

Most DOT exam sites can only check boxes. If your blood pressure or blood sugar is out of range, they hand you a short-term certificate and tell you to see your doctor. Lani City Medical is a full medical clinic with urgent care and primary care under one roof, so the provider who found the problem can start treating it the same visit.   • Glasses, contacts, or hearing aids you use while driving
  • List of current medications with dosages
  • Diabetes: recent A1c and, if on insulin, the MCSA-5870 form from your treating provider
  • Sleep apnea: CPAP compliance report from the last 30 to 90 days
  • Heart conditions: most recent stress test or cardiology clearance
  • Any employer forms that need to be completed
